Registration 

2026 LAVA BEAR WATER POLO REGISTRATION CHECKLIST

To ensure your player is completely registered for the 2026 water polo season, please make sure the following items are completed:

1. $500 Registration paid to Lava Bear Water Polo via Stack Team App (click STORE to register) or check payable to Lava Bear Water Polo (players must also turn in additional fundraising amounts prior to playoffs - $600/$400 for siblings).
   Registration is due NO LATER THAN the first day of practice.

2. Copy of physical turned in to Board President/VP or emailed to lavabearwaterpolo@gmail.com .  Physicals are good for two years.

3. Lava Bear Water Polo Policy and Media Consent signature - read thoroughly.

4. Concussion Acknowledgement signature.

5. Purchase team suit via "Store" in Stack Team App if needed (same uniforms as 2025). 

6. Dual Sports Agreement completed and signed IF participating in more than one sport

7. Join USA Water Polo (Basic Bronze membership only, $65)
